The vital statistics block of the résumé should contain the following information:

1. YOUR NAME:
   • Is written in a larger type font (the most visually prominent information on the page)
   • Uses your full name--not a nickname
   • May use an initial for first or middle name


2. ADDRESS:
   • If possible presents only one address
   • If using both a permanent and current address, balances the page by placing them side by side
   • Establishes the address format you will use for all other addresses


3. TELEPHONE NUMBER:
   • Includes area code and establishes format you will follow for references' telephone numbers if you are asked to include a list of references


4. E-MAIL ADDRESS:
   • May either be centered under your name and address if you list only one address, centered under your name if you list two addresses and have access to your e-mail from both locations, or listed below your telephone number if you only have access from one location. (Omit the e-mail address if you don't check your e-mail regularly.)
