1. Please be sure to read and complete the informed consent form (provides your permission) and to assist your child in completing the minor assent script (child gives their permission) that will follow once you click "Continue."
2. Print a copy of both forms for your records before you hit the "Submit" button.
3. After both forms have been completed, you will be directed to the BEES.
4. Please read carefully and follow all instructions.
5. Remember to select your BEES Code on the top of every page.
